You Register
All registrants to AMCIS 2008 must be current members of AIS.
To find out the status of your membership or to join AIS, visit
the AIS website at www.aisnet.org
or contact the AIS HQ at membership@aisnet.org.

How
to Register
There are three easy ways to register for AMCIS 2008. Payment must accompany form, in US Funds Only. Purchase Orders are not accepted. Checks should be payable to “AIS.”
1.
Register Online
Complete your online registration registration. This is a secure web server, so your registration information and credit card number is protected.
2. Register by Fax
Simply download and complete the registration form and fax to (404) 413-7443. Fax registrations must include payment by credit card.
3. Register by Mail
Simply
download the registration form and mail the completed form
and your payment to the address below.

If
paying by check, mail your check and registration form before
July 28, 2008 to ensure proper processing.
Registration Amendments
To amend your registration (add a guest, register for a workshop), send an email to registrar@aisnet.org, indicating the changes to be made or call (404) 413-7442 for assistance.

Travel
Documents
Passport Requirements:
It is strongly advised that all visitors to Canada have passports.
This facilitates entry into Canada and entry back into the visitor’s
home country. The United States now requires that all airline
travelers have passports, including US citizens, to enter the
US (please see this website for more details).
Visa Requirements:
There may be some requirements for visitors coming to Canada
in terms of visas. The visa requirements are described in more
detail on the following website: http://www.cic.gc.ca/english/visit/visas.asp
If
you require a supporting letter to attend AMCIS 08 in Toronto,
you will need to download the form here,
complete it and send to AIS at registrar@aisnet.org.
Substitutions
& Cancellations Policy
Before registering, please read through AIS’s policy regarding
conference substitutions and cancellations.
Substitutions
& Cancellation Policy
Request for refund or cancellation must be in writing via email,
fax or mail and are subject to the following schedule: received
by June 13, full refund less $25 administrative fee; received
by July 25, full refund less $50 administrative fee; cancellations
received by the AIS headquarters on July 26, or later will not
be accepted and the registrant will be liable for the full conference
registration fee. Substitutions for current AIS members are
permitted.
Registrants
who fail to attend or notify the AIS Membership Coordinator
at membership@aisnet.org of cancellation by the July 26 date
are subject to the full fee and refunds WILL NOT
be granted. NO EXCEPTIONS.
